Dear Valued Customer,

The situation with the coronavirus (COVID-19) continues to develop and we know it is having a significant impact on our San Joaquin County community. Last week the County of San Joaquin declared a State of Emergency and the City of Stockton issued a local emergency related to COVID-19.

To reduce the risk for participants who are at a higher risk for severe illness from COVID-19 and our employees, Active Life will be temporarily suspending in-person services at our Adult Day Care facility effective 3/18/2020 based on guidance from our Governor, the Centers for Disease Control and Prevention and local public health officials. Active Life will continue to follow the recommended guidelines set forth by the Governor and state public health officials regarding mass gatherings to help limit unknown exposure to the disease and will notify all participants, physicians, Managed Care plans, vendors, and staff upon reopening. Currently, we anticipate on or after 4/13/2020.

Active Life ADHC is a licensed and Medi-Cal certified CBAS Program (Community Based Adult Services) center that provides medical and social services in a daytime setting for adults ages 18 and up, including the frail elderly. We work closely with caregivers and family members to help our program participants:

stay living at home

delay institutionalization

minimize hospital visits

This vital community-based program provides valuable respite to families and caregivers who can drop off their loved ones and clients for four hours of supervised activities and therapy each day of attendance.

Our person-centered approach to providing care helps our participants feel engaged and involved in maintaining their own health in ways they prefer. We empower them to voice their preferences in all aspects of daily life at the center.

We receive referrals daily from caregivers, family members and medical offices who know individuals that would benefit from our program. Once we receive a referral, it begins a process that triggers us to learn more about the individual candidate to see if they're eligible for the program and to see if we can safely serve them.

We will evaluate your needs
We will interview you to see if you will qualify and benefit from CBAS services. We will also determine if our facility is well suited to safely serving your needs.

We will confirm your eligibility
We will confirm if you are eligible to receive our services at No Cost to you through a Medi-Cal Managed Care Plan such as Health Plan of San Joaquin or HealthNet. Private Pay options are available upon request.

We will work with your primary care physician
We will work with your physician to better understand how we can help with your individual needs and to obtain any documentation needed to bill your managed care plan for your services.

We provide transportation
If you require transportation, we provide it! Our vans provide door-to-door service, and our drivers provide assistance to those who need it.

Eligibility
This program is available to eligible Medi-Cal beneficiaries who are enrolled in a Medi-Cal managed care plan such as Health Plan of San Joaquin, Health Net®, Kaiser / Medi-Cal or others.

This benefit is called Community Based Adult Services (CBAS). Eligibility to participate in a CBAS program is determined by the beneficiary's Medi-Cal managed care plan.

Eligible candidates are persons 18 years of age, older adults and frail elderly with chronic disabilities, including cognitive, medical, mental, and physical conditions, placing them at risk of institutional care.

Activities and Lunch Room
Our newly remodeled facility includes a large area where our participants can come together for group activities, games, arts and crafts, and to receive their hot and nutritious lunches.

Computer Lab
We provide our participants with access to computers so that they can do things like check their email, catch up on social media, browse the internet, read current news and to support their interests.

Feel at home
We design our spaces, like this reading nook, to help you feel more at home in our center.

Exercise/Physical Therapy Room
We provide you with the equipment and expertise you need to help you maintain your strength, remain independent and be confident.

Spacious courtyard
Enjoy lush green surroundings in a safe, enclosed courtyard while you socialize, work on an arts and crafts project, or participate in a group exercise activity.
