  • Active Life ADHC is a licensed and Medi-Cal certified CBAS Program (Community Based Adult Services) center that provides medical and social services in a daytime setting for adults ages 18 and up, including the frail elderly. We work closely with caregivers and family members to help our program participants:

  • stay living at home
  • delay institutionalization
  • minimize hospital visits

  • This vital community-based program provides valuable respite to families and caregivers who can drop off their loved ones and clients for four hours of supervised activities and therapy each day of attendance.

    Our person-centered approach to providing care helps our participants feel engaged and involved in maintaining their own health in ways they prefer. We empower them to voice their preferences in all aspects of daily life at the center.

    We receive referrals daily from caregivers, family members and medical offices who know individuals that would benefit from our program. Once we receive a referral, it begins a process that triggers us to learn more about the individual candidate to see if they're eligible for the program and to see if we can safely serve them.

    We will confirm if you are eligible to receive our services at No Cost to you through a Medi-Cal Managed Care Plan such as Health Plan of San Joaquin or HealthNet. Private Pay options are available upon request.

    We will work with your physician to better understand how we can help with your individual needs and to obtain any documentation needed to bill your managed care plan for your services.

    Eligibility
    This program is available to eligible Medi-Cal beneficiaries who are enrolled in a Medi-Cal managed care plan such as Health Plan of San Joaquin, Health Net®, Kaiser / Medi-Cal or others.

    This benefit is called Community Based Adult Services (CBAS). Eligibility to participate in a CBAS program is determined by the beneficiary's Medi-Cal managed care plan.

    Eligible candidates are persons 18 years of age, older adults and frail elderly with chronic disabilities, including cognitive, medical, mental, and physical conditions, placing them at risk of institutional care.
